Keep your health summary in one place.
A one-page summary of your key health information, ready to hand over in an emergency, a hospital admission or when you see a new care team.
When you are unwell or meeting a new team, it is hard to remember every condition, medicine and allergy. Keeping them on one page means the people treating you can see them quickly.

Questions
What is the health summary for?
It puts the most important facts about your health on one page, so a new doctor, nurse or emergency team can see them quickly.
Can I use it outside the UK?
Yes. The sheet has a field for an NHS number, but you can leave it blank or write another health ID, such as an insurance member number.
Where should I keep it?
Somewhere safe that you and the people close to you can find quickly, such as with your medicines or in the bag you take to appointments. Update it whenever something changes.
Can I print it?
Yes. It is an A4 PDF, designed to be printed. If your printer uses US Letter paper, choose “Fit to page” in the print settings.
Is this medical advice?
No. The sheet helps you record information and prepare for appointments. It does not diagnose or treat anything, and it does not replace advice from a healthcare professional. If symptoms are severe or getting worse quickly, seek medical help.
