What it helps you do
- See a whole month of symptoms on one page
- Score each symptom the same way every day, from 1 (slight) to 5 (severe)
- Capture symptoms that change between morning and evening
- Keep sleep, medication and possible triggers next to your symptoms
- Bring the notes that matter most to your doctor
How to use it
- 1Write your symptoms down the left: steady symptoms in rows 1–10, and symptoms that change through the day in rows 11–20.
- 2Each day, score every symptom from 1 to 5, or leave it blank if you didn't have it. For symptoms that change, put the morning score top left and the evening score bottom right.
- 3Fill in the daily life rows: impact on daily life (1–3), hours of sleep and whether you took your medication.
- 4Log anything that might have triggered a change, and before your appointment, write what worries you most in Notes for my doctor.

Questions
What is the symptom tracker for?
It gives you one consistent way to record your symptoms each day for a month, so changes and patterns are easier to spot and explain at an appointment.
Can I print it?
Yes. It is an A4 landscape PDF, designed to be printed. If your printer uses US Letter paper, choose “Fit to page” in the print settings.
Can I use it for any condition?
Yes. It works for any condition or set of symptoms, including chronic illness, flares, pain, fatigue, migraine and changes after starting a new treatment.
Is this medical advice?
No. The sheet helps you record information and prepare for appointments. It does not diagnose or treat anything, and it does not replace advice from a healthcare professional. If symptoms are severe or getting worse quickly, seek medical help.
