FREE PRINTABLE SHEET

Walk into your appointment prepared.

A one-page sheet to fill in before you see a doctor, so your main concern, symptoms and history are ready when the appointment starts.

Appointments are short, and it is easy to forget details in the moment. Writing them down beforehand helps you cover what matters most.

Questions

What is the appointment prep sheet for?

It helps you organise what you want to say before an appointment, so your main concern, symptoms and history are clear and easy to share.

When should I fill it in?

A day or two before your appointment works well, while you have time to think. Add anything new on the day and take the sheet with you.

Can I record my appointment too?

Many people do, so they can remember what was said. Always ask first. Rules differ between countries and US states, so see recording your appointment for details.

Can I print it?

Yes. It is an A4 PDF, designed to be printed. If your printer uses US Letter paper, choose “Fit to page” in the print settings.

Is this medical advice?

No. The sheet helps you record information and prepare for appointments. It does not diagnose or treat anything, and it does not replace advice from a healthcare professional. If symptoms are severe or getting worse quickly, seek medical help.
