Questions
What is the after-appointment checklist for?
It helps you check that you understand your diagnosis, treatment, tests and next steps, and collects anything you are unsure about into one follow-up list.
When should I use it?
As soon as you can after an appointment, while it is fresh. You can also take it with you and work through it before you leave the room.
Can I print it?
Yes. It is an A4 PDF, designed to be printed. If your printer uses US Letter paper, choose “Fit to page” in the print settings.
Is this medical advice?
No. The sheet helps you record information and prepare for appointments. It does not diagnose or treat anything, and it does not replace advice from a healthcare professional. If symptoms are severe or getting worse quickly, seek medical help.
